Job Description: Nurse Practitioner - Hormone Therapy Telehealth Clinic

Position Overview:

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our Hormone Therapy Telehealth Clinic. In this role, you will provide high-quality telemedicine services to patients seeking hormone therapy, ensuring individualized care and support for their unique health needs.

Key Responsibilities:

Conduct comprehensive telehealth assessments and evaluations for patients seeking hormone therapy.
Develop and implement personalized treatment plans based on patient needs, medical history, and current health status.
Monitor patient progress and adjust hormone therapy dosages as needed, providing ongoing education and support to enhance patient outcomes.
Collaborate with patients to educate them about hormone therapy, potential side effects, and lifestyle modifications.
Perform virtual follow-up appointments to assess treatment effectiveness and address any concerns or questions.
Maintain accurate and up-to-date medical records in compliance with clinic policies and legal regulations.
Work collaboratively with a multidisciplinary team to ensure cohesive patient care and referral as necessary.
Stay current with best practices, advancements in hormone therapy, and telehealth technologies to provide the most effective care.

Qualifications:

Master’s Degree in Nursing with Nurse Practitioner (NP) certification.
Active and unrestricted Nurse Practitioner license in the state of practice.
Previous experience in hormone therapy, endocrinology, or related field preferred.
Strong communication and interpersonal skills, with a focus on patient education and support.
Proficient in telehealth technologies and electronic medical record systems.
Ability to work independently while also being a team player.

Preferred Skills:

Additional certification in hormone therapy or related specialties is a plus.
Experience with patient-centered care and a holistic approach to health.
