Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients across various age groups, focusing on mental health and medication assisted treatment. This role is essential in delivering high-quality healthcare services in a Residential Reentry Center environment, ensuring that patients receive the best possible care.

Duties

Conduct thorough patient assessments and diagnostic evaluations to determine health status.
Develop and implement individualized treatment plans based on patient needs and evidence-based practices.
Ordering of needed laboratory tests.
Manage patient care, collaborating with multidisciplinary teams to ensure optimal patient outcomes.
Educate patients on health maintenance, disease prevention, and management strategies.
Maintain accurate and up-to-date patient records in compliance with healthcare regulations.

Skills

Proficient in diagnostic evaluation within a resdiential reentry center setting.
Strong background in mental health and substance abuse treatment and prevention..
Experience working in detox and rehabilitation is a plus
Excellent communication skills to interact effectively with patients and healthcare professionals.
Ability to work collaboratively within a multidisciplinary team to enhance patient care delivery.
Strong organizational skills to manage multiple patient cases efficiently while maintaining attention to detail.

Pay: $58.00 - $116.00 per hour
