**Job Overview:**

We are seeking a dedicated Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care and will be comfortable with post acute patients. This is a full time position and you will be seeing 8-10 patients a day while you provide telehealth visits from the comfort of your home. Make a difference in the lives of patients who have been recently ill and who cannot easily leave their home to see healthcare providers. We focus on our patients and collaborate with many other groups to make sure that our patients are comfortable in their own home.

**Responsibilities:**

- Telehealth visits with patients who have recently discharged from a facility or who are homebound

- Conduct patient assessments and develop appropriate care plans

- Interpret test results

- Evaluate for health risks and provide advice to reduce those risks

- Collaborate with physicians and other healthcare professionals

**Requirements:**

- Valid Nurse Practitioner license

- Experience in internal medicine or family practice and patient assessment

- Strong communication and interpersonal skills

- Commitment to continuous learning and professional development

Pay: $134,000.00 - $150,000.00 per year

Expected hours: 40 – 45 per week
