Family Nurse Practitioner

Telehealth/Remote position with occasional in-home patient appointments (Must live in the US)

Primary population: Adult population with chronic conditions focusing on preventative and restorative health

FiaCare provides medical care to a primarily adult population through app and telehealth outreach to patients principally in the Salt Lake and Utah Valley areas. We are looking to build a team of enthusiastic providers who are aligned with our vision to reduce costs, simplify, and improve access to healthcare.

Brief overview of responsibilities:

-Provide primary care services to patients with chronic conditions, providing outreach care through messaging, phone, telehealth and occasional in home visits.

-Conduct tele-health visits for patient with acute medical needs.

-Collaborate with field RN/Case managers to create patient care plans to include best practice treatment, education, and connection to community services

-Help patients manage patient’s current medications, provide prescription counseling, and patient education

- Ensure patient information, visit history, outgoing referrals, lab follow-ups, phone calls, and consults are properly documented in an HER

-A current, valid driver's license and good driving record is required

-Physical requirements may include the ability to lift-up to 50 lbs

Qualifications

Licensure: Family Nurse Practitioner Certification, APRN and RN licensure per the UT Board of Nursing. Maintain current BLS

Experience: Prefer 1+ year of family practice, urgent care, orthopedic, OBYN, or dermatology. Will consider new graduates

Education: Graduate of an accredited Family Nurse Practitioner Program. Master’s Degree required.

Pay: $90,000.00 - $130,000.00 per year
