Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join our Remote Patient Monitoring healthcare team. The ideal candidate will provide high-quality patient care to seniors, working collaboratively with physicians and other healthcare professionals. This role involves assessing, diagnosing, and treating patients, with a focus on delivering comprehensive care to individuals with high blood pressure

Responsibilities

Conduct thorough patient assessments and physical examinations.
Develop and implement individualized care plans based on patient needs.
Collaborate with patients' PCP to enhance patient outcomes.
Maintain accurate and up-to-date medical records
Ensure compliance with HIPAA regulations to protect patient confidentiality.

Qualifications

Master’s degree in Nursing or equivalent advanced practice nursing degree.
Current Nurse Practitioner licensure in the state of Georgia
Experience in geriatric care is highly desirable.
Familiarity with electronic health record systems.
Excellent communication skills and the ability to work effectively within a team environment.

Job Type: Part-time

Pay: $60.00 - $65.00 per hour

Expected hours: 5 – 8 per week
