Responsibilities
The Oncology Nurse Navigator is responsible for coordinating patient care efficiently, addressing barriers to treatment, and enhancing patient outcomes. This role involves assessing patients' physical, psychological, and social needs, providing expert communication, and facilitating access to care. Key responsibilities include:
- Conducting patient assessments and transitioning care based on individual needs.
- Educating and advising patients while advocating for their health care needs.
- Collaborating with healthcare providers to manage patient care effectively.
- Documenting interactions and developing care plans in electronic medical records.
- Participating in committee work and data analysis for survivorship programs.
Qualifications include a BSN, New Mexico Nursing License, BLS certification, and at least 5 years of healthcare experience, preferably in oncology.
