Warm Hearth at Home —Home Health and Hospice is seeking a dynamic and compassionate clinical professional to become the agency's Director of Nursing (DON) for hospice services .
The DON will oversee, manage, and direct the agency's hospice nursing services, working both in the field and in the office to ensure that our clients receive the care and support they need.
This role requires keen attention to detail, a passion for excellent customer service, and a desire to engage with clients, families, caregivers, and colleagues in a compassionate setting for end-of-life care .
- Ensures, in conjunction with the Director/Administrator, that an adequate quantity and quality of staff are available and scheduled according to the patient's needs.
- Is responsible for the quality of all services and compassionate care provided by the agency.
- Supervises all staff of the hospice program
- Assists with the recruitment and hiring of professional personnel.
- Ensures compliance with all state, federal, and local laws regarding licensure and certification of staff.
- Participate and provide leadership in the team for regular interdisciplinary group meetings; the development, review, and revision of the patient/family plan of care; facilitation of problem-solving sessions; and delivery of team support.
- Implement and monitor compliance of policies and procedures.
- Ensures proper maintenance of medical records as prescribed and in compliance with local, state, and federal laws.
- Oversees clinical supervision of nursing personnel, support team, and evaluation of performance.
- Promotes h ospice referrals in the community.
- Stays informed about changes in the field of nursing and h ospice and disseminates information with staff and administrative personnel.
- Identifies the need for staff education and assists in the development of in-service programs for interdisciplinary groups.
- Participates in a QAPI program
- Participates in on-call and clinical rotations.
- Other duties as assigned by the Director/Administrator.
