Administrative Nurse Care Coordinator Remote Philadelphia FIGHT was initially founded in 1990 as a comprehensive AIDS service organization. Today we are a Federally Qualified Health Center (FQHC) with clinical sites that provide state-of-the-art culturally competent primary care, advocacy, social services, consumer education and outreach for patients in need regardless of HIV or insurance status.The patients and staff at Philadelphia FIGHT are diverse and represent a broad spectrum of race, religion, gender identity, sexual orientation, HIV status, and physical ability. Summary: We are seeking an experienced administrative nurse to join our dynamic team of health professionals. The ideal candidate will take a trauma-informed and harm-reduction approach to health care. The ideal candidate will also be proficient in Spanish.The John Bell Health Center and Jonathan Lax Treatment Center provide a safe space for people to access medical care, regardless of the individual's insurance status, ability to pay, or life circumstances. Our team of compassionate providers focus on addressing each patient's unique needs and challenges. Both Clinics offer comprehensive HIV primary care.
Qualifications: To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills and ability required.Professional Education/Experience:
- Bachelor's degree required
- General knowledge base in science, medicine, substance abuse, and the running of a medical clinic. Demonstrate the ability to understand the overall purpose of clinical care and importance of support staff.
- Registered Nurse in the state of Pennsylvania
- Within 30 days of hire, complete training in Blood borne pathogens, Post exposure prophylaxis, and HIPAA regulations.
- Prior to shipping blood specimens, complete IATA training for shipping hazardous substances, if applicable
Other Skills:
- Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors.
- Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in multi-task environment.
- Needs to have attention to detail, dependability and flexibility.
Essential Duties: Essential job duties for this position include the following items. Other duties must be performed as assigned or required. Must complete in-person training for the first month and attend in person All Health Center staff meetings once a month.Clinical:
- Triage sick/emergency walk-ins and sick calls and new patients (take full medical history and determine urgency of illness) and communicate to medical providers
- Communicate normal and abnormal labs to patients and providers
- Assist with one-on-one management and teaching of complicated patients with multiple diagnoses, complicated medical regimens, and/or complex social needs
- Work in collaboration with the PrEP (Pre-Exposure Prophylaxis for HIV) team, to provide HIV prevention to at-risk patients, the C a Difference (Hepatitis C) team to provide access to Hepatitis C evaluation and treatment for infected patients, and the staff at Prevention Point to coordinate care for patients at John Bell and Jonathan lax Center
- Identify and advocate for patients needing urgent or ongoing behavioral health services and/or substance abuse treatment via collaboration with the primary provider and/or onsite Behavioral Health Consultant.
- Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, medical case managers, pharmacy technicians, nutritionists and behavioral health specialists
- Demonstrate commitment to clinical care and confidentiality
Administrative:
- Assist providers with obtaining urgent reports for diagnostic exams, specialist consults, and hospitalizations
- Assist providers in scheduling imaging studies and procedures, particularly those requiring prior authorization
- Assist providers in making specialist appointments for patients who need additional assistance
- Assist providers with prior authorizations for medications including specialty medications such as Hepatitis C treatments, Suboxone, and Vivitrol (an injectable opiate blocking agent) in conjunction with the pharmacy technician
- Organize daily clinical huddle with providers, nurses, and other support staff
- Track high risk and complex patients using excel spreadsheets (i.e., patients on Coumadin, Suboxone, or Vivitrol etc.)
- Order and coordinate delivery for durable medical equipment for both John Bell and Lax
- Report and assist in communicating appropriate information on reportable diseases to oversight bodies, e.g., Public Health Department, AIDS Activities Coordination Office
- Participate in monthly Continuous Quality Improvement committee
- Interact closely with the other clinicians in the practice, Practice Coordinator, and Medical Director, by participating in regular meetings for purposes of addressing management issues and quality assurance.
- Assist and support outreach and recruitment initiatives, in addition to educational programs as necessary.
- Alongside other Philadelphia FIGHT staff, participate in regular cultural competency trainings to best serve our diverse patient population.
- Exercise good judgment in bringing any issues necessary to the attention of the Medical Directors of the John Bell Treatment Center and or the Chief Quality Officer of Philadelphia FIGHT.
- Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, behavioral health specialists, community health workers, pharmacy technicians, medical case managers, practice coordinators, and medical providers
Other Tasks:
- Coordination of Care - Provide consultation and coordinate care of patients with clinic staff at the John Bell Health Center and at Jonathan Lax Center
- Charting - Complete all charting and documentation as needed and required by contracted funding sources, including assessment, progress notes, and administrative documentation
- Billing - Complete all billing documentation as needed and required by contracted funding sources, including billing codes and required documentation
- Supervision - Attend all John Bell Health Center and Johnathan Lax Center team meetings, including Hepatitis C "Huddle" and attend FIGHT case conference weekly with all clinicians
Physical Demands: The ability to sit continuously and to work at a computer screen for extended periods of time. Must be able to see, determine, identify, recognize, and inspect. Must be able to operate, use, prepare, place, position computer equipment and/or laboratory equipment.
Other: Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.
Salary: $87k, Exempt
