POSITION SUMMARY:
The healthcare professional will primarily provide outpatient rehabilitation and medical services in an in-home setting, outpatient clinic, adult day center, or long-term care facility for the older adult population.
Job Responsibilities: Physical Therapist (PT)
✔ Oversee patient care and treatment.
✔ Collaborate with other healthcare professionals.
✔ Provide a safe and compassionate environment.
Physical Therapy Assistant (PTA)
✔ Assist physical therapists in providing therapy services.
✔ Monitor patient progress and report results.
✔ Educate patients on condition management at home.
Occupational Therapist (OT)
✔ Evaluate patients' occupational needs and limitations.
✔ Develop individualized treatment plans to improve daily activities.
✔ Work with healthcare professionals for comprehensive care.
Occupational Therapy Assistant (COTA)
✔ Assist occupational therapists in providing therapy services.
✔ Document progress and report concerns.
✔ Educate patients and families on condition management.
Registered Nurse (RN) / Licensed Practical Nurse (LPN)
✔ Provide skilled nursing care and treatment planning.
✔ Monitor patient health status and administer medications.
✔ Educate patients and families on managing chronic conditions.
Speech-Language Pathologist (SLP)
✔ Evaluate and treat speech, language, and swallowing disorders.
✔ Develop personalized therapy programs.
✔ Provide guidance to caregivers on communication techniques.
Social Worker (SW)
✔ Assess patient social and emotional needs.
✔ Provide resources and support for patients and families.
✔ Advocate for patient rights and coordinate care services.

What is Required?
✔ Valid PT/PTA/OT/COTA/RN/LPN/SLP/SW license as required by the state.
✔ CPR Certification.
