REGISTERED NURSE ( RN ) CASE MANAGER HOME HEALTH JOB SUMMARY
A Registered Nurse ( RN ) Case Manager provides and directs skilled nursing care to a group of Home Care patients. Assesses patients and their home situation, communicates with physicians and other disciplines involved, provides care and education to patients and caregivers, and acts as a liaison to facilitate patient care needs.
REGISTERED NURSE ( RN ) CASE MANAGER JOB SPECIFICATIONS
Education and/or experience: Licensed as a registered nurse in the State of Tennessee. RN experience of 2 years preferred. Home care or emergency care experience preferred.
Skills/Abilities: Requires strong assessment and communication skills, organization and time management. Must be self-directed. Must be available for after-hours call rotation. Must be able to provide direct patient care to all assigned patients.
