Weight Loss clinic is seeking family medicine Nurse Practitioners with the ability to prescribe to join our team to fulfill its mission of reducing obeasitity through comprehensive treatments. We provide care on an outpatient basis primarily to those struggling with weight loss, as one of the most passionate providers in the state.
We offer evidence based medication-assisted weight loss treatment to offer the most effective level of care to the patients that we serve.
Essential Duties And Responsibilities
Provide initial evaluation, diagnosis, treatment and ongoing support for individuals with weight loss concerns and related health conditions
Complete full and appropriate documentation to support professional billing for services
Provide education regarding illness and prognosis to assist with clarification of patient and family goals of care and support development of a comprehensive care plan
Interpreting test results to diagnosis any health issues and provide feedback to patients
Educating and Counseling patients regarding medication compliance
Work with an interdisciplinary team of professionals to administer the best care for those patients that need not only medical therapy but behavioral health services as well.
Patient follow-up visits regarding patients' progress with medication and/or treatment
Contacts patient's PCP and/or specialist(s) when necessary
Evaluates lab results and provides consultation with APPs regarding abnormal results
Reviews and oversees progress and treatment plans, including making any adjustments
Maintains required licensure, certifications and CME credits
Minimum Qualifications
Active and unrestricted medical license as a nurse practitioner within the states of Delaware, Maryland, Ohio, Pennsylvania, New Jersey, West Virginia or Virginia, Florida, New York, Idaho.
Experience providing medication management services or both medically and socially complex populations; Experience managing complex disorders.
Ability to work with diverse care teams in a variety of settings
Effective time management and communication skills
Computer literate and experience with Electronic Medical Records

Pay: $40.00 - $100.00 per hour
