Provide medically appropriate detoxification services to clients that includes addiction plan implementation, monitoring, and assessment; general medical monitoring of clients; referral of clients to outpatient/ inpatient treatment programs and/or self-help program when appropriate; assist with overall duties of the PHD.
1. Provide direct care and delegate care of clients as indicated by client needs.
2. Administer medications and treatment as directed by the standing orders and/or phone orders from attending Licensed Health Care Professional.
3. Observe and record pertinent clinical information in regards to the care and condition of the client.
4. Assesses patient's physical and psychological condition on an ongoing basis throughout the continuum of care
5. Formulate individualized nursing care plans and participate in client care conferences.
6. Dispense and record administration of medications according to prescription instructions and agency policy.
7. In conjunction with the addictions team, make referrals to appropriate hospitals and agencies based on the client's needs.
8. Give and receive report at staff shift change.
9. Consult with other health team/care professionals for guidance in the development of the most appropriate health care plan for the client.
10. Confer with and assist the licensed Health Care Professional.
11. Skills to assess symptoms of detoxification using standardized measures such as CIWA or COWS.
12. Attend and participate in all in-service or training sessions.
13. Maintain all required training and continuing education necessary for licensing.
14. Maintain standards of ethical conduct.
15. Observe and maintain client confidentiality & privacy laws according to 42 CFR regulations. I 6. Must be willing to work a flexible schedule. Holiday and work during inclement weather will be required
