JOB DESCRIPTION - CERTIFIED REGISTERED NURSE PRACTITIONER
Established Health Care Center in Philadelphia, PA
Quality Community Health Center, a FQHC operating for over 40 years, provides service at 3 locations in Philadelphia, PA managing the health and wellness of patients from infants through senior citizens. We practice medicine as a partnership between us and our patients. Currently, we are seeking a full-time Certified Nurse Practitioner to provide comprehensive acute and chronic care to all adult and pediatric patients.
Essential functions:
- Provide primary care services to Pediatric and Adult patients.
- Assist in the triage of all patients presented to the clinic for services.
- Ensure that all protocol for patient care and clinic operations are adhered to.
- Work closely with the Adult and Pediatric Physician to ensure that all services are available to all patients.
- Under the supervision of the Adult and Pediatric Physician, perform health evaluations for all patients who are presented to the clinic.
- Provide emergency assistance when necessary.
- Maintain logs of all patients who are screened or medically evaluated.
- Counsel parents and teachers when necessary, on aspects of child’s physical and mental health.
- Ensure that all patient records are maintained in the prescribed manner.
- Ensure patient confidentiality at all times.
- Emergency telephone coverage according to rotation
- Other duties as assigned by Clinic Director
- Adherence to OSHA and CLIA regulations.
- Participate in administering immunizations, protocols, and procedures as indicated by the ACP and ACIP.
- Participate in weekly conferences.
- Participation in chart audits, as assigned. Adhered to QCHC House Rules. Participate in the develop, implement and ensure adherence to QCHC’s
- Bioterrorism Plan, according to guidelines as set forth by the Pennsylvania
- Department of Health and the Center for Disease Control.
PERFORMANCE MEASUREMENTS
1. Maintain and/or improve health outcome of patients seen.
2. Provide service to 100% of patients seen.
3. 100% adherence to protocols both clinical and patient care.
4. Provision of appropriate services to all patients served.
5. 100% participation in communication with Adult and Pediatric Physician
for direct patient care.
6. 100% participation in emergency measures as indicated.
7. Maintain logs of patients with 100% accuracy.
8. 100% participation in counseling parents and teachers.
9. 100% maintenance of patients records as prescribed.
10. Maintain patient confidentiality 100% of the time.
11. 100% adherence to OSHA and CLIA standards.
12. 100% adherence to protocols and procedures according to ACP and ACIP.
13. 100% participation in conferences.
14. 100% participation in chart audits as assigned.
15. 100% compliance to QCHC policies and protocols.
