JOB DESCRIPTION
Title: Nursing Assistant
THIS POSITION IS ONLY FOR CERTIFIED NURSE ASSISTANTS.
Purpose of Your Position:
The primary purpose of your job position is to provide routine nursing care in accordance with our established policies and procedures, and as may be directed by the Unit Charge Nurse, Director of Nursing, or Administrator, to assure that the highest degree of quality patient care can be maintained at all times.
Major Duties and Responsibilities:
- Receive assignments and check your patient schedule.
- Report on and off duty.
- Verify the identification of a patient before administering treatment, serving food, etc. to assure that appropriate treatment, etc. is being provided to the correct patient.
- Perform all duties assigned.
- Assist patients with dental and mouth care.
- Assist patients with identifying food arrangements (i.e. informing blind patients of where food is located on tray.)
- Assist with trays and feeding of patients. Remove trays when finished.
- Perform after meal care as required (cleaning patient’s hands, face, clothing, etc.)
- Assist in preparing patients for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting foods, supervision in dining room, etc.)
- Assure that patient’s food is accessible and self-help devices are available as needed.
- Record patient’s food intake as directed.
- Feed patients who cannot feed themselves.
- Serve extra nourishment in accordance with established nursing care procedures.
- Serve drinking water as required. Assures that the water pitchers are cleaned and filled with fresh drinking water daily where appropriate.
- Bathe patients as assigned and in accordance with our established nursing care procedures (encourages showers and other self-help measure/activities.)
- Dress patients neatly, and in their own clothing. Assists patients in dressing as necessary.
- Comb and brush patient’s hair daily. Shampoo as instructed.
- Clip and trim fingernails, etc. as directed.
- Shave male / female patients daily or as instructed.
- Keep face on male / female patients clean and shaven as necessary or as desired by resident.
- Give back care as instructed.
- Observe and report the presence of pressure areas to prevent bed-sores.
- Assist patients to / from bathroom. Offer bed-fast patients bed-pans, urinals, portable toilet, etc. Clean and store such equipment after each use. (Report intake output as directed.)
- Keep incontinent patients as clean and dry as possible at all times. Change bed linens and clothing as often as necessary.
- Check and report bowel movements and character of stools as directed.
- Test urine for sugar and acetone as well as collect specimen stools, sputum, etc. as directed.
- Make beds, clean bedside tables, clean bedrails, etc., as well as other housekeeping duties that relate to nursing care procedures.
- Assist patients in preparing for test, x-rays, therapy, activities, physician visits, religious services, etc., to include transporting patients to / from such activities.
- Assist in therapy treatment as instructed.
- Move and turn bed-fast patients at least every two (2) to three (3) hours. Record such information as directed.
- Assist patients into chairs, beds, bathroom, bathtubs, showers, etc.
- Report all complaints to the Charge Nurse.
- Assist patients to walk with or without self-help devices as necessary.
- Perform restorative and rehabilitative procedures as necessary.
- Report any patient leaving the premises, or who has left the premises, to the Charge Nurse immediately.
- Assure that the established smoking regulations are followed by personnel, visitors, patients, and the general public. Report violations to the Charge Nurse immediately. Do not permit residents to smoke in bed.
- Assure that the Nurse call system is attached to the bed when residents are bedfast and within easy reach at all times.
- Assist others in heavy lifting. Use proper equipment.
- Answer patient’s calls promptly.
- Report any patient abuse immediately (i.e. harsh / abusive language, unnecessary roughness, etc.)
- Watch for and report any change in room temperature ventilation, lighting, etc.
- Keep work areas clean at all times.
- Wash hands after caring for each patient.
- Follow infection control procedures, as established by this facility, when isolation techniques are necessary.
- Perform routine nursing care such as colostomy bags and drainage, etc. as directed by the Charge Nurse.
- Chart required information as instructed.
- Ensure that patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage a complaint, are followed at all times.
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit.
- Maintain confidentiality of all pertinent patient care information to assure patients rights are protected.
- Report all accidents/incidents, regardless of how minor, to the Charge Nurse as soon as possible.
- Report all hazardous conditions and equipment to the Charge Nurse immediately.
- Check for food in patient’s room. Discard as outlined within our nursing care procedures.
- Attend and participate in in-service educational programs, on the job training programs, etc. as directed.
- Assure that work/assignment areas are clean and equipment, tools, supplies, etc. are properly stored before leaving such areas on breaks, end of work day, etc.
- Other related duties as may become necessary or as directed by the Charge Nurse, Nursing Supervisor, Nursing Director or Administrator.
