Job Summary:
Commonwealth Pain and Spine is looking for a passionate, tech savvy LPN to serve in this crucial role of delivering high-quality remote nursing services to our patients will contribute to enhancing patient outcomes and ensuring seamless remote care delivery. This role will report to our Corporate Office location with the option for a hybrid working model. You will work closely with our interdisciplinary team of healthcare professionals to monitor patients’ physiological data and provide timely interventions as needed and empower them to make informed decisions about their healthcare, manage chronic conditions, and achieve their health goals.

Key Responsibilities:
Assist in refining and improving remote monitoring workflows and protocols to enhance the overall quality and efficiency of our remote nursing services.
Patient Engagement:
- Establish and maintain a strong rapport with patients, promoting trust and open communication to help coach them to improved health through goals and education on self-management strategies.
- Utilize our customized remote monitoring platform to review and analyze patients’ physiological data, including pain scores, vital signs, activity levels, and other relevant metrics.
- Conduct thorough assessments of patients’ health conditions based on the collected data, identifying any potential changes or abnormalities.
- Provide patients with regular check-ins to monitor progress and provide ongoing support.
Health Education/ Care Coordination:
- Support the delivery of provider ordered care plans for each patient, incorporating remote monitoring data and the patient’s medical history.
- Communicate directly with patients via video calls, phone calls, or secure messaging to provide support, education, and guidance on managing their health conditions.
- Promptly escalate any concerning trends or critical data to the patient’s provider for further evaluation and decision-making.
- Maintain accurate and up-to-date patient records within our electronic health records (EHR) system, ensuring compliance with privacy regulations.
Empowerment and Motivation:
- Support patients in setting realistic health and wellness goals.
- Monitor progress toward goals and provide positive reinforcement and motivation.
- Assist patients in overcoming barriers to achieving their health objectives.
Data Management:
- Maintain accurate and up-to-date patient records and care plans.
- Document all patient interactions and interventions in compliance with relevant regulations.
- Generate reports and track patient outcomes to measure program effectiveness.
Patient Advocacy:
- Advocate for the patients’ needs and rights within the healthcare system.
- Assist patients in navigating insurance, financial assistance, and community resources.
Compliance and Training:
- Stay current on healthcare regulations, industry trends, and best practices.
- Attend training sessions and participate in continuous learning and development.
Requirements:
Qualifications/Education:
- Current, unrestricted Licensed Practical Nursing License.
- A deep understanding of medical care, strong communication skills, and a compassionate approach to patient support.
- Strong clinical background with experience in remote patient monitoring, telehealth, or similar settings.
- Proficiency in using remote monitoring platforms and electronic health records systems.
- Experience in and a passion for healthcare and improving patient outcomes.
- Excellent communication skills, both written and verbal, to effectively interact with patients and the medical team remotely.
- Ability to critically analyze physiological data to make informed nursing judgments.
- Empathetic and compassionate approach to patient care, understanding the unique challenges of remote healthcare delivery.
- Strong organizational skills and attention to detail to maintain accurate patient records and documentation.
- Familiarity with HIPAA regulations and patient privacy standards
- Proficiency in electronic health records (HER) and healthcare software.
- Some flexibility in work hours may be necessary to accommodate patient needs, including evenings or weekends.
