The part-time School-Based Health Center Nurse Practitioner manages the School-based Health Center in a high school and provides school nursing and primary care services to students in the assigned high school. The Nurse Practitioner provides preventive and acute care, chronic care management, and reproductive health care, supervising a Licensed Practical Nurse to deliver school nursing in compliance with all State mandates and coordinate primary care delivered by the Nurse Practitioner, responsible for billing and reporting requirements, and all appropriate documentation. He/she reports to the Deputy Director, with accountability to the Directors of Administration and Health, with support and oversight by a collaborating physician.

Responsibilities:
· Responsible for the delivery of high-quality care and practice outcomes in nurse-managed school-based health center/s serving low-income student populations.
· Provide preventive physical and behavioral health screenings and care coordination as appropriate.
· Provide primary care services, including medication management, diagnosis and prescription, chronic care management and coordination in collaboration with PCP/specialists, preventive well-child check-ups, reproductive health in the high schools.
· Provide special focus, screening, and care management related to SBHC priorities: asthma, healthy weight, behavioral health, and sexual health.
· Direct and deliver lab tests, chronic health assessments, and coordinate all follow-up care as needed.
· Provide supervision of LPN School Nurse Assistant, strengths-based guidance and feedback, and annual evaluation, task supervision to community health worker.
· Provide oversight and responsibility of health center facilities and equipment, in communication and collaboration with Director, Education Plus Health.
· Document & bill all services in electronic medical record athenahealth within required timelines.
· Establish health care plans of students with special health care needs and update them as needed.
· Maintain comprehensive health records on each child.
· Complete monthly and annual reports as required by Education Plus Health, the school and the Department of Health.
· Perform all documentation for billing in accordance with agency policy.
· Ensure provision of all clinical services according to the policies and procedures, applicable professional standards, facility and program licensure as required, laws, rules and regulations.
· Provide overall management of assigned School Based Health Center/s serving as primary liaison to principal and school leadership and staff for health center administration and student care coordination.
· Work with external partners to provide special services as appropriate.
· Identify high risk children for needed services, including students presenting signs of child abuse.
· Work with the Director to develop and actualize strategic vision related to school-based community health care.
· Actively support and facilitate school and all community partnerships.

Qualifications/Requirements
¨ Licensed Nurse Practitioner in Pennsylvania with 2-3years pediatric experience at minimum.
¨ Board Certification preferred as a Nurse Practitioner in pediatric or family medicine, adolescent health experience preferred.
¨ Pursuit of school nurse certification.
¨ Family planning experience a plus
¨ Pennsylvania Registered Nurse license
¨ Masters Degree in Nursing (MSN) and/or Doctor of Nursing Practice (DNP)
¨ School-based health experience a plus.
¨ Required State and Federal clearances
