Job Title: Staff Nurse
Responsible to: Director of Nursing
Minimum Qualifications
- Education: Licensed Nurse graduate from an accredited or state approved School of Nursing with current license to practice in the State of Indiana
- Knowledge and Abilities: Have a positive attitude working with the elderly. Must be flexible and tactful in planning maximum independence of resident functioning. Must be able to work under varying degrees of supervision and maintain open lines of communication with the Director of Nursing or designee. Must be able to assess resident health status, plan, implement and evaluate nursing care and contribute to the plan of care for the residents. Must have the ability to organize nursing care and communicate effectively with physicians and pharmacy regarding resident needs and necessary orders. Must have knowledge of facility policies in order to ensure compliance with State regulations.
Physical Requirements
- During shift scheduled, must be able to stand/walk/sit the length of the shift
- Must be able to use hands for repetitive grasping, pushing/pulling and fine manipulation
- Must be able to bend, squat and lift frequently during the length of the shift
- Must be able to lift 75-100 lbs occasionally, 35-50 lbs frequently with occasional sitting and frequent standing/walking
- Other physical requirements may require two-person lifts/transfer or use of mechanical lift
Area of Responsibility
- Provide direct care to residents
- When necessary, provide direct resident care
- Administer medications and/or treatments to residents, adhering to policy and procedures
- Implement safety measures, infection control measures, preventative measures and display correct aseptic technique when providing nursing care
- Deliver resident care as directed by the residents plan of care
- Coordinate, supervise and evaluate resident care
- Supervise nursing assistants
- Visit and evaluate each resident, according to assignment, to monitor resident condition and response to treatment
- Assure goals and approaches from plan of care are followed; convey to CNA verbally and via CNA Kardex then ensure plan of care is implemented
- Initiate plan of care upon admission, update plan of care as needed; ensure updates are implemented
- Observe direct care administered by staff ensuring safe and effective care is provided in a timely manner as well as completion of other assigned duties i.e. BM record, daily intake forms, prn medication log, MDS grids etc.
- Supervise and assure completion of daily, MDS and Q-shift documentation is completed in the medical record; ensure follow-up documentation is completed as well as lab results, new orders etc.
- Contribute topics for in-services
- Make physician appointments and arrange for transportation
- Recognize performance and conduct in regards to facility policy and standards to ensure retention of qualified staff
- Accompany physicians during resident rounds and assist with examinations and completion of visit documentation
- Complete admission and discharge process ensuring that required documentation and assessments are completed in a timely manner as determined by regulation
- Participate in Quality Assurance meetings as requested
- Perform tasks as indicated on the Staff Nurse orientation check list
- Attend and participate in initial orientation and regularly scheduled in-services.
- Attend and participate in mandatory in-services as assigned.
