Overview
The RN Case Manager plays a crucial role in coordinating and managing patient care, often working with patients with complex medical needs. Their primary goal is to ensure that patients receive the appropriate care and services while managing costs and improving outcomes.
Responsibilities
- Conduct comprehensive patient assessments to determine needs and develop individualized care plans.
- Coordinate and monitor care across the continuum, including hospital, rehabilitation, home care, and outpatient services.
- Serve as a liaison between patients, families, physicians, and other healthcare providers to ensure optimal patient care.
- Perform utilization reviews, ensuring that services provided are cost-effective and medically necessary.
- Facilitate patient education and discharge planning to promote continuity of care.
- Communicate with insurance providers to obtain authorizations and ensure reimbursement for services.
- Advocate for patients’ needs, ensuring they receive the appropriate resources and support.
- Maintain up-to-date documentation in compliance with regulatory standards.
Unit Specific Details
Shift Monday-Friday 8a-4:30p with one weekend per month required
*Manager will be interviewing for this opening after clinical, this may delay an offer*
Requirements
- Active RN license in VA
- Min. one year acute care hospital experience required -will consider IRF, LTAC but prefer acute care experience
- 3 Years discharge planning preferred
- EPIC, CarePort Parachute experience preferred
- Must have Mother/baby experience
