The Medical Liaison is primarily responsible for the daily coordination of all medical and mental health appointments for persons served by Tri-Ko, Inc. The Medical Liaison will schedule appointments, complete all paperwork, and obtain necessary consent forms. Responsible for various duties in the Medical Services Department serving both the medical transportation and nursing divisions. Will ensure that all verbal and written communication is delivered to appropriate health/mental health professionals and returned to Medical Services Department staff following appointments. Complete confidentiality is required. Must be responsive to the needs of the Medical Services Department. Responsible for on-call hours as assigned if medically certified or licensed, as needed. Employee will perform other tasks as assigned. The Medical Liaison is directly responsible to the Director of Medical Services.
Responsibilities:
Responsible for medical and mental health appointments. Schedules and organizes appointments, staffing, transportation, driving directions, completes and updates all paperwork (referrals, intake papers, patient history, insurance info, consents, etc.), verifies insurance coverage with healthcare agencies for specific procedures for persons served with private and public insurance, assures funding available for medical services not covered by insurance, obtains guardian consent as needed, and informs guardians regarding appointments and necessary follow-up after the appointment. Maintains a running calendar of appointments and issues weekly calendar of persons served medical appointments for department managers.
Responsible for the safe transportation of individuals served to and from medical and mental health appointments.
Attend medical and mental health appointments advocating for the person served. Maintain professional communication between the various medical/mental health professionals and Tri-Ko by assuring that referral forms are taken to all appointments and returned.
Coordinate delivery and pickup of durable medical equipment for repair and maintenance including but not limited to special medical equipment, hearing aids, and glasses- ordering and sending for repairs. Location and accessing any donation resources as needed.
Assist with monthly medication change-over by reviewing all medications for accuracy and comparison to the new eMARs and any administration of medications as needed.
Provide personal direct care such as dietary, toileting, stimulus, etc., as needed.
Assist with lunch staffing, complete goals; assist with general transportation and transitions, etc., as needed.
Participate in team meetings, attend Person Centered Support Plan (PCSP’s) meetings, etc, as needed.
Complete area maintenance, housekeeping, medical vehicle inspections, and medical vehicle detailing/cleaning, as needed/assigned.
Must deliberately and accurately complete all designated documentation, i.e. medical referrals, nurses notes, goals, supports, attendance, daily logs, etc.
The Medical Liaison will be expected to work the floor when there is any down time in the Medical Services Department. The Medical Liaison will provide responsive, safe, and meaningful assistance to persons receiving Day Services from Tri-Ko, Inc. The Medical Liaison will provide direct support that includes but is not limited to supervision, coaching, teaching, prompting, as well as more complex and intimate care including dietary, hygiene, toileting assistance, and the following.
Assist persons served to increase their independence and to exercise their rights, by teaching skills in various activities of daily living (self-care, care of the household, communication, leisure activities, vocational activities, etc.) according to both formal plans and as natural teaching opportunities arise, using informal teaching techniques/procedures;
Assist persons served to reduce undesirable behaviors and to learn appropriate replacement behaviors by implementing approved behavior management procedures, according to formal plans and as natural teaching opportunities arise;
Assist in maintaining the health, hygiene and grooming of persons served by teaching new skills, implementing first aid as needed, health care and hygiene techniques according to instructions or plans and, if properly certified, by administering medications according to stated policies and procedures;
Documenting care/assistance provided, behavior management and teaching techniques implemented and compliance with various health and active treatment standards through accurate and timely completion of daily logs, checklists, records and schedules;
Provide active and meaningful support by continually engaging persons served in age-appropriate, functional activities and implementing community access activities (self-government, family-style meals, etc.);
Punctual attendance and completion of assigned shifts and flexibility and willingness to fulfill assignments at all work sites depending upon needs of the persons served.
Will always promote dignity and respect, and will foster an environment of kindness and optimism, always free from abuse, neglect, and exploitation.
Additional duties may include general transportation, housekeeping, and other duties as appointed, etc.
ACCOUNTABILITY
This Medical Liaison position will be directly responsible to and may be assigned additional duties by the Director of Medical Services.
QUALIFICATIONS
High School Diploma/GED preferred, but not required. Certified Medication Aid (CMA) preferred, but not required. Must be at least 21 years of age. One - two years of experience in a position of similar scope, preferred, but not required. Valid Driver’s License or CDL Driver’s License with Passenger Endorsement, Physical or Medical Examiner’s Certificate, and the ability to be covered by corporate vehicle insurance. Must meet all requirements necessary for transporting persons served.
Must have the ability to:
· Perform unassisted transfers of disabled adults with adaptive equipment (gait belts, Hoyer lifts, etc.);
· learn and perform basic safety techniques (which include lifting, bending, kneeling, stooping, reaching, blocking, grabbing);
· recognize natural teaching opportunities and act upon them;
· handle all aspects of personal care with sensitivity and caring;
· redirect undesirable (sometimes aggressive) behaviors while maintaining calm and composure;
· be physically and mentally capable of providing safe transportation in corporate vehicles;
· report for work on site, on time, work for the full shift, and be flexible regarding work location;
· communicate through verbal, written and electronic mediums;
· observe, ask questions, listen actively, and give and accept feedback effectively;
· Maintain a high energy level, adaptability, assertiveness, patience and ability to work in a team environment.
TRAINING REQUIREMENTS
Attendance and completion of annual and safety training courses and certification in Wheelchair Securement, Mandt, Medication Administration, CPR, and First Aid. Further educational/ certification requirements may be assigned.
HAZARDS, RISKS, DISCOMFORTS OF PARTICULAR JOB
The Medical Liaison may perform tasks, which on occasion may potentially expose the employee to blood borne pathogens, infectious materials, physical and/or verbal aggression. Employee may be required to perform tasks involving heavy lifting or bending. May be exposed to raw materials and/or chemicals in the work area. May operate motor vehicles or equipment including but not limited to computer, fax, photocopier, calculator, typewriter, telephone, paper shredder, cooking stove, microwave, audio/visual appliances, golf cart, pallet jacks, forklift, shrink wrap machine, box knives, handcart, heat sealer, hand tools, wheelchair lift, transfer lift.
PERSONAL PROTECTIVE EQUIPMENT
Protective gloves, gowns, face masks, shields, goggles, work gloves and hazardous waste disposal bags. Tri-Ko will endeavor to provide individual training, protective equipment and/or ergonomic equipment aids, or accommodations as needed to prevent hazards that may relate to tasks required of this position.
POSITION CLASSIFICATION/HOURS OF WORK/BENEFITS
This position is classified as a non-exempt position. Wages will be paid at an hourly rate. The general hours of work are 8:00 a.m. – 4:00 p.m. Monday through Friday, scheduled for 40 hours per week. Hours may vary as tasks require. Full employee benefits are available for this position.
