Location: Akron, OH
Position Type: Full-time (40 hrs/week)
Compensation: $142,500/year base + 10% bonus potential. Incentive of $30K lump sum after 36 months, plus a $10K base salary adjustment. $2K annual allowance for continuing education and recertification.
About the Role:
We are looking for a compassionate, experienced Nurse Practitioner (NP) with a commitment to transforming care for individuals living with Serious Mental Illness (SMI). In this role, you will lead the medical aspects of a dedicated team, conducting whole-person assessments, supporting care coordination, and working closely with behavioral health providers. Home visits are a key part of the job, as we believe in meeting patients where they are to build trust and provide accessible care.
Responsibilities:
- Perform Comprehensive Assessments: Conduct in-depth evaluations of physical and behavioral health, gather information from other care providers, and document findings to support disability applications when appropriate.
- Lead Team-Based Care: Provide clinical guidance and support to Guides (peers) and Community Resource Guides (social workers), assist in care coordination, perform medication reconciliations, and facilitate effective communication with community health centers, FQHCs, and hospitals.
- Care Management: Identify and address actionable care gaps, provide individualized care recommendations, and coordinate referrals to programs and services that meet patients' needs.
Qualifications:
- Education: Master’s degree or higher in Nursing.
- Experience: 5+ years of clinical experience as an NP, ideally in a primary care setting; experience with underserved populations and SMI preferred.
- Licensure: Valid Nurse Practitioner (NP) license in Ohio.
- Preferred Skills: HCC coding experience, knowledge of trauma-informed care, familiarity with social determinants of health, and connections within the local community.
