```TAKING CARE OF PATIENTS IN THEIR HOMES`
- Provide personal care services to patients in their homes
- Assist with activities of daily living, including bathing, dressing, and grooming
- Administer medication as directed by healthcare professionals
- Monitor and record vital signs, such as blood pressure and temperature
- Assist with mobility and transfers, using proper lifting techniques
- Help patients with memory care and dementia care needs
- Maintain a clean and safe environment for patients
- Follow infection control procedures and maintain HIPAA compliance
- Document patient care activities and report any changes in condition to the supervising nurse
```Skills```
- Experience in memory care and dementia care is preferred
- Knowledge of medical terminology and ability to understand medical records
- Ability to provide compassionate and patient-centered care
- Strong communication skills to effectively interact with patients, families, and healthcare professionals
- Ability to lift and transfer patients safely using proper body mechanics
- Familiarity with healthcare systems and protocols
