Key Responsibilities:
- Conduct home visits to assess and create personalized care plans.
- Train and support caregivers, ensuring they can meet their loved ones' needs.
- Monitor and report on clients’ health, coordinating with their primary care physicians.
- Facilitate smooth transitions and ongoing care for clients.
- Maintain compliance with all relevant regulations and standards.
- Full time Hybrid position offering autonomy while utilizing your nursing assessment, care planning and case management skill
What You Will Bring
- Must hold an active Registered Nurse (RN) License in the state of employment
- Minimum two (2) years of recent experience in direct care of elders or adults with disabilities
- Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
- Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems)
- Experience with Minimum Data Set (MDS) assessment tool a plus.
- Assess client status upon initial home visit to identify needs in order to develop a comprehensive case management plan.
- Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting nursing and related needs.
- Document in an accurate and timely manner. Maintain a clinical record for each morbidity patient.
- Completing the MDS-HC and other nursing assessments while coordinating all other applicable clinical assessments.
- Training, evaluating, and supervising AFC caregivers in conjunction with the care manager.
- Monitoring the health status of all members to ensure that all needed AFC is properly delivered.
- Proficiency in common software and web-based applications including Microsoft Excel, Word, PowerPoint, and Teams. Ability to input data into a universal portal.
- Additional state-specific qualifications may be required.
