Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in various nursing settings and possess a strong commitment to providing high-quality patient care. As an LPN, you will play a crucial role in supporting patients' health and well-being, particularly in home care, senior care, and critical care environments.
Duties
- Provide direct patient care under the supervision of registered nurses or physicians.
- Administer medications and treatments as prescribed by healthcare providers.
- Maintain accurate patient records, including documentation of care provided.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Ensure compliance with healthcare regulations and safety protocols.
- Assist with Certified Residential Medication Administration (CRMA) training to all employees.
- Assist in program visits to support medical needs for consumers and provide program specific training to Direct Support Professionals.
- Assist with medication administration audits and supporting the nursing department with corrective action plans to reduce medication errors and or deficiencies
- Engage with consumers, guardians, and other team members regarding medical needs.
- Keep current with LPN ‘best practice’ standards and various state requirements.
Qualifications
- Valid LPN license in the state of practice.
- Experience in home care, occupational health, or hospital medicine preferred.
- Knowledge of DME (Durable Medical Equipment) usage is a plus.
- Familiarity with ICD-9 coding is beneficial for accurate documentation.
- Strong skills in family planning and triage processes.
- Experience in critical care or senior care settings is highly desirable.
- Excellent communication skills and the ability to work well within a team environment.

Pay: $55,000.00 - $60,000.00 per year
Expected hours: 32 – 40 per week
