We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team.
The full time shift is : 2:00 a.m. to 10:00 a.m. typically Monday through Friday. Weekends as needed. (please see the pay incentives above for these hours)
As an LPN, you will play a crucial role in providing high-quality care to the individuals with developmental and intellectual disabilities who live here.
Performs all the duties of a Staff Nurse in the Department of Nursing, providing nursing care to all residents under the auspices of the Director of Nursing and staff physicians.
Provides nursing assessments of employees who are injured or become ill while on the job and administers employer-provided tests and inoculations.
Receives and gives verbal or written reports on residents.
Receives, transcribes, and verifies physician orders. Acting on a physician’s order, administers and charts medications, indicating dosage, route, and frequency. Initials/signs Kardex and verifies new Kardex at the beginning of the month.
Completes pharmacy orders; orders medications from pharmacy and verifies receipt. Maintains medications and supplies.
Makes chart entries in nursing notes, including, but not necessarily limited to:
· Vital statistics (Wt. qm; BP, P, R, Wt., Ht. Quarterly)
· Incidents observed (i.e. unusual incidents)
· First seven (7) days after admission
· Special appliances; use and progress
· Medication changes; effects and side effects
· PRN’s use, effects and side effects as appropriate
· Treatments (i.e. size of sore, healing progress; must be measurable)
· Behavior at medication tx. Administered; progress, effects and side effects
· Temperature of person on antibiotics; effects and side effects
· Lab work; obtain as ordered with review upon receipt; advises physician of any unusual development or complication
· Diet modifications (i.e. changes and follow-up)
· Intervention as described in the Plan of Care and monthly progress of goals
Assures availability of supplies necessary to complete collection of various specimens for laboratory study.
Admits and orients new residents and staff to Nursing Services. Assists with discharge and transfer of residents, including completion of the discharge summary.
Assesses need and schedules appointments for various specialists, lab, dental, xray, vision, and medical exams and assists in obtaining transportation for the resident. Maintains accurate records, including follow-up on these.
Updates residents’ Plan of Care as necessary, but at least quarterly.
· Prepares necessary reports, such as annual and quarterly medical reviews, and presents this information to other health care members at appropriate times or when deemed necessary.
Provides 24-hour “On Call,” first aid, and consultative services.
Keeps nursing area (office) neat and clean.
· Maintains necessary equipment used for various treatments, procedures, and assessments.
· Orders supplies from Dietary and Housekeeping as needed.
Assists with physicals as needed. Assists private physician and consulting physician, and reports follow-up information to attending physician and/or R.N. as appropriate prn.
Maintains frequent contact with residents to observe behavior and progress and to note residents who are ill or have special problems. Assesses nursing intervention needs and documents response. Performs skilled nursing care prn.
Assists as an active member of the health care team to meet state and federal regulations and standards as applied to Intermediate Care Facilities (ICF).
· Communicates appropriately with other nursing staff, physicians, case workers, child care workers, or other interested parties regarding health-related information when it is instrumental to the well-being of a resident.
Coordinates and assists in educating residents on personal hygiene and grooming and provides assistance to and functionally supervises direct care workers in providing general health care (i.e. temperature, pulse).
· Assists in providing periodic, updated health-related, in-service education and maintains appropriate records, documenting types of in-service education and those involved.
Is aware of and practices the legal aspects of nursing care as related to ICFMR’s and is responsible for knowing and implementing policies and procedures affecting Flat Rock Nursing Services.
Attends pertinent meetings directly related to the physical and mental health of residents as needed (i.e. monthly team, program coordination, all staff, special consults with the behavior specialist, quarterly reviews, and special team meetings).
Assesses on-the-job injuries or illnesses incurred by employees. Completes required paperwork, administers first aid as appropriate, and/or refers employees to outside health care.
Administers annual TB screening for all employees and maintains records. Administers Hepatitis B vaccines if requested by employees, and maintains records.
Serves on committees, such as Sanitation and Infection Control, Human Sexuality, Safety, etc.
Attends all required meetings, in-services, and training programs.
MARGINAL FUNCTIONS
Performs other related duties as may be assigned.
May train employees in first aide and CPR, if qualified, approved, and necessary.
REQUIREMENTS
Must hold a current license for a practical nurse (LPN) in the State of Ohio. Must have a high school diploma or equivalent and be at least 18 years of age. Must be able to communicate effectively with residents, co-workers, and supervisors. Must be able to complete nursing reports and have basic math skills. Must have sound judgement so as to be able to ensure the health and safety of our residents. Must be able to problem-solve, follow physicians’ and supervisors’ directions, and be able to work independently as well as part of a team. Must have compassion, patience, and tolerance for persons with differing abilities. Must be able to perform each of the essential functions and the physical demands of the position with or without reasonable accommodation. Prior experience in working with individuals who are mentally, physically, and/or developmentally challenged is highly desirable. Must enjoy working with children. Must hold a valid driver’s license in the State of Ohio and have an acceptable driving record.
PHYSICAL DEMANDS
Must be able to perform the essential functions of the position with or without reasonable accommodation. Must be able to perform behavior intervention techniques.
WORK ENVIRONMENT
Service is delivered using a team approach. The persons served sometimes display aggressive or combative behavior that may result in injury to an employee. Work is performed in a Christian oriented environment.
