Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will provide direct patient care under the supervision of a registered nurse or physician. Your primary responsibility will be to coordinate OP clinics, document an accurate record of the patients course of treatment based on the plan of care and disease process.
Duties:
1. Organizes outpatient clinics to meet provider, patient, and department needs.
· Develops and distributes clinic schedule to appropriate staff in a timely manner.
· Contacts patients to communicate appointment schedule.
· Directs and assists outpatients to proper department for tests.
· Organizes, notifies, or distributes outpatient faxes to the proper departments.
· Provides education materials and explanation of upcoming procedures to patient and/or family.
· Maintains individual patient files with appropriate documentation.
· Audits and completes documentation to provide information needed for charging and billing for services.
· Reconciles charges for services and reports information to billing department for accuracy of billing process.
· Orders department supplies to maintain inventory level to meet patient needs.
· Maintains patient education materials for distribution.
2. Performs specific nursing care based on individualized nursing and medical plan of care.
· Performs patient assessment in accordance with the applicable patient type, which may include: Medical, Surgical, or Pediatric patients.
· Identifies abnormal symptoms/changes in patients clinical, mental, or emotional condition reporting findings to the RN.
· Includes patient, family, and support system in implementing and education of interventions.
· Performs patient procedures under the guidance of the physician or RN or as determined by policy and standard of care.
· Assists with patient admission and discharge processes as directed by the RN and within scope of practice guidelines.
· Provides and explains dismissal instructions verbally and with written instruction sheets as directed by provider or RN
· Assists with monthly review of services.
· Measures and monitors quality assurance for department.
3. Maintains and performs all patient documentation in a timely, factual, and legible manner.
· Utilizes only approved abbreviation in all documentation.
· Accurately reflects nursing care provided.
· Follows all policies and procedures related to documentation requirements.
4. Performs Emergency treatment as required
· Initiates or assists in patient care in an effective and responsive manner as each situation requires.
· Administers and performs emergent care in accordance with the applicable patient type which may range from newborn to geriatric
· Follows emergency policies and procedure and standards of BLS.
5. Promotes and ensures patient safety in all aspects of responsibilities.
· Follows procedures and protocols to ensure safe administration of mediations and monitoring for adverse effects.
· Utilizes proper body mechanics, transfer/lifting techniques to minimize fall risk to patient and injury to self.
· Transport’s patients in a safe manner using safety standards such as side rails and safety strap.
· If assigned by RN, the LPN will be responsible to Conduct defibrillator function test the day of service, will maintain test strip, and ensure emergency cart is locked.
· Implements infectious disease controls as appropriate including isolation procedures, proper use of hand washing and glove, specimen collection and transport and during cleaning processes of rooms or equipment/instruments.
· Follows proper procedures for wasting narcotics and disposal of sharps or hazardous materials.
Perform all other duties as assigned.
Qualifications:
- Must have a valid LPN license in Missouri
- Previous experience in a healthcare setting preferred
- BLS certification required. If certifications are not current upon hire, must be completed with in six months
- IV Therapy Certification
