Job Overview:
We are seeking a skilled and compassionate Nurse Practitioner to join our adult primary care office. As a Nurse Practitioner, you will need to be able to work independently and in collaboration with memebers of the healthcare team to provide patient-centered care. We are located in the Champlain Valley in Northern NY. Responsibilities:
- Perform comprehensive physical examinations on patients
- Diagnose and treat acute and chronic illnesses
- Develop and implement treatment plans in collaboration with the patient and health care team
- Provide health coaching and education to patients
- Monitor patient progress and adjust treatment plans as necessary
- Collaborate with interdisciplinary healthcare team members to ensure coordinated patient care
- Document patient information accurately and thoroughly using electronic medical record systems

Qualifications:
- Valid Nurse Practitioner license in New York or be eligible for reciprocity
- Previous experience in family/adult primary care or urgent care preferred
- Proficiency in computer and EHR use
- Strong communication and interpersonal skills
- Ability to work effectively as part of a team

Pay: $65.00 - $85.00 per hour

Expected hours: 32 – 40 per week
