IMMEDIATE HIRE FOR HALL & SURROUNDING COUNTIES

We are looking for a qualified registered nurse or licensed practical nurse to promote and restore patients’ health. You will work with a variety of patients and you will conduct nursing assessments to identify the patient's level of care for Medicaid benefit approval.

Responsibilities

Identify patients’ care requirements, focus on their needs and capture those needs on the assessment.
Conduct timely documentation and communicate any challenges in a timely fashion.
Sign off on Level of Care forms and return them timely to the client’s file.
Provide education to the client and family members of programs available through the Medicaid Elderly and Disabled Waiver Program.
Follow care regulations and standards
Work within and cooperate with a multidisciplinary team

Skills

Proven nursing experience, preferably in Long Term Care
Ability to use a variety of e-documentation systems with training
Problem solving skills and ability to multi-task
Compassionate with good communication skills
Excellent teamwork skills
BS or diploma in nursing

Experience

Must be licensed to practice nursing within the state of Georgia (Compact license is acceptable)
MDS Experience strongly preferred
Experience in long term care required

Meeting deadlines and completing documentation timely is a MUST, non-negotiable. Assessments are completed in person.

$75-$100 PER VISIT plus travel pay

Pay: $1,000.00 - $3,000.00 per month

Expected hours: 10 – 25 per week
