REPORTS TO: Administrator/Director of Nursing/RN Clinical Supervisor

JOB SUMMARY: The licensed practical nurse(LPN) is responsible for making decisions that are based on the individual’s educational preparation and nursing experience and for providing nursing services as ordered on patient plan of care. Reports problems/potential problems to RN Clinical Manager.

DUTIES AND RESPONSIBILITIES:

Participation in the nursing process by providing input to the client’s plan of care

The LPN will be expected to implement the care plan as written

Evaluating effectiveness of care plan and make recommendations for revisions as appropriate

Records all pertinent observations and treatments as per Haven Home Health policies. Prepares clinical progress notes and provide appropriate documentation of service in keeping with Haven Home Health policies and procedures

Provide for the emotional and physical comfort and safety of client taking into consideration their rights and cultural background

Participate in case conferences, team meetings, staff meetings and performance improvement activities as assigned

Provides proof of current license by state. Provides necessary information to verify education and work experiences

Practices safely and competently within the job description of the LPN

Complies with all state regulatory acts

Adheres to all Haven Home Health policies and procedures including but not limited to the HIPAA privacy rule

Provides documentation of high-tech certification, if appropriate and performs duties as qualified/authorized

QUALIFICATIONS:

1. Must be a graduate of an approved school of nursing and currently licensed in the state of the Haven Home Health.

2. Must have one (1) year current experience in accredited hospital or accredited nursing home within two (2) years previous to employment

3. Provide a copy of LPN license/renewal
