O'Connell Care at Home is looking for a per-diem Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team at our Greenfield location serving the greater Franklin County area.

Duties and Responsibilities (including but not limited to):

Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines.
Complete new client assessments, care plans, and updates as necessary.
Completes in home demonstrations of personal care services for clients with community based caregivers
Medication management and occasional skilled work with clients in the place that they call home
Follows company policies and practices
Performs additional related duties as assigned.

Minimum Requirements

Possess a current and unencumbered Massachusetts license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Graduate of an accredited LPN or RN program
Minimum of two years' experience in the health care field; homecare preferred
Possess a positive attitude, a willingness to learn, with a desire to be an effective member of the team
Excellent communication skills and the ability to build meaningful relationships with internal and external stakeholders
Comfortability using various forms of technology and software (Wellsky)
Cognitive skills as related to the position.
Thorough understanding of Private Case vs ASAP Cases and their requirements
Due to the nature of our business or a myriad of other factors, employees may need to work from home. For that reason, access to reliable secure internet service at home is required.
Reliable transportation, current driver’s license, and ability to drive to our various locations as well as the locations of our clients and stakeholders on a regular basis.
