JOB TITLE: LICENSED PRACTICAL NURSE

JOB DISCRIPTION: Under supervision of the Administrative Services Coordinator and the Clinical

Coordinator, provides supervision and direction in the care of the residents.

QUALIFICATIONS:

1. Shall be a licensed practical nurse, with valid state credentials.

2. Shall have three to five years nursing experience and at least one-year experience working

with elderly or chronically impaired adults.

3. Demonstrates competence in working with the impaired elderly and knowledgeable of their

physical, social, and mental health needs, as well as knowledge and skills to work with

residents with dementia.

4. Able to make decisions, to work independently, to exercise good judgment.

5. Must be flexible, have emotional stability; acceptance of people; non-judgmental, non-

prejudicial; and is highly motivated.

RESPONSIBLITIES:

1. Assists in monitoring residents for disease progression and managing behaviors. Provides

medications and treatments per physician’s orders.

2. Offers personal cares and ADL assistance to residents using the Best Friends approach.

3. Monitors and reviews written orders from physicians for all residents. Assists in making new

MAR’s and Dr. Visit Forms as needed.

4. Communicates with physicians regarding changes in resident’s status as needed per

direction of the resident or their responsible party.

5. Assists with the development and updating of resident care plans.

6. Assists in reviewing health assessment and monthly summaries of each resident.

7. May assist with performing employment health screening on new staff.

8. May assist with the direction, supervision, and monitoring of medication aides.

9. Coordinates with pharmacy to set up medications as ordered by physician for the residents

as needed.

10. Monitors documentation of administration of medications and treatments for residents.

11. Maintains records of CEU’s in accordance with prevailing professional standards.
