Description:

Provide services to residents in an Assisted living community.

RN/LPN:

Assess patients’ condition during every visit and chart your observations
Perform evaluation tasks, including vital signs and medication review
Administer medication as prescribed by the residents Physician
Dress or redress open wounds and assess progress of healing
Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan
Listen to the patient and respond to concerns or requests.
Regularly reevaluates the client needs, and coordinates the necessary services.
Initiates the Plan of Care and necessary revisions and updates to the plan of care and the care plan.
Informs the physician and other personnel of changes in the client condition and needs.
Counsels the client and family/caregivers in meeting their needs.
Prepares clinical and progress notes. Documents changes in client condition and actions taken, updates care plans, obtains physician orders, and communicates verbally and in writing to members of the team.
Reports findings and observations to the registered nurse and other members of the team to assure coordination and timely response to client changes or needs. Reports changes directly to the RN and documents the findings and the reporting in client record.
Prepares clinical and progress notes.
Observation and assessment, teaching and training activities. Management and evaluation of the care plan and routine and complex skilled procedures.

Required Skills

The nurse will demonstrate competency in providing procedures such as:

a. Insertion and irrigation of urinary catheters and foley.

b. Nurse needs to be familiar with Point-Click-Care (PCC).

c. Consistent availability is required.

Pay: LPN $42.00 - $46.00 & RN $55.00 - $60.00 per hour
