These communities are located in Williamsburg, Gloucester, & Newport News. We have Full Time positions. New grads welcome for those with acute and chronic disease management experience.

Mid-Atlantic Long Term Care, a provider owned company, is seeking qualified Nurse Practitioners to join our practice in the beautiful Hampton Roads region of Virginia. We are a group of over 130 providers currently serving over 140 Long Term Care Facilities. We have serviced Skilled Nursing Facilities, Assisted Living, and Continuous Care Retirement Communities for over 20 years.

We pride ourselves in providing a high quality of care to our residents and facilities.

The ideal candidate will have long term care experience, however extensive clinical experience in Family Practice, Internal Medicine or Hospital may be adequate for a motivated individual. For those with clinical experience, but no long term care experience, we provide training regarding the regulatory and clinical model of long term care.

Qualifications:

Requires licensing in the State of Virginia. Experience working with the geriatric population (preferred).

Experience collaborating as part of a multidisciplinary team
Basic knowledge of Microsoft Office and experience using Electronic Medical Records systems.
Must possess effective communication skills to effectively interact with patients, families, and facility staff

Responsibilities
- Conduct patient assessments and provide primary care support
- Utilize EMR and EHR systems for accurate patient documentation
- Collaborate with multidisciplinary teams for comprehensive patient care
- Demonstrate excellent communication skills with patients and families
- Apply physiology knowledge in patient care and treatment plans
- Ensure high-quality care in a medical setting with a focus on internal medicine

Job Types: Full-time, Part-time, PRN

Pay: $95,000.00 - $165,000.00 per year
