We are seeking a dedicated and passionate Family Nurse Practitioner (FNP) to join our team remotely as a full-time/part-time 1099 contractor. This role offers the flexibility to set your own hours and work from the comfort of your home while making a significant impact on improving health disparities. Multi-state licensure is preferred.

**Key Responsibilities:**

Conduct virtual primary care evaluations and assessments.
Manage and oversee care for chronic diseases, acute care needs, and routine health maintenance.
Develop and implement individualized treatment plans.
Collaborate with a multidisciplinary team to provide holistic care.
Maintain accurate and timely documentation in our electronic health records system.
Engage in ongoing education and training to stay current with best practices in telehealth and culturally competent care.

**Qualifications:**

Active FNP certification.
Licensure in multiple states is preferred.
Strong commitment to minority health and caring for culturally and ethnically diverse populations.
Excellent communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Experience with telehealth platforms is a plus.
