We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will have a passion for patient care and possess the necessary skills to provide high-quality medical services.

Duties
- Provide direct patient care in outpatient settings
- Conduct patient assessments and assist in developing care plans
- Implement infection control procedures to maintain a safe environment
- Collaborate with healthcare professionals in various specialties
- Manage and administer medications as prescribed
- Assist in case management and patient education

Skills
- Proficiency in patient assessment and care delivery
- Experience in long-term care
- Ability to work effectively with other disciplines

- Strong understanding patient care
- Familiarity with case management principles
