Summary
As a Case Manager at Tribal Health, you will play a vital role in coordinating patient care and services for individuals in underserved communities. This position is essential in advancing our mission to improve healthcare access and outcomes, making a significant impact on the lives of patients and their families.
Responsibilities
- Needs assessment: Evaluating a patient's social, emotional, and financial situation to identify potential barriers to healthcare access and treatment adherence.
- Care coordination: Connecting patients with appropriate community resources like housing assistance, transportation, food banks, or mental health services based on their needs.
- Discharge planning: Assisting patients with transitioning from inpatient to outpatient care, including coordinating follow-up appointments and home care services.
- Advocacy: Representing the patient's needs and advocating for necessary services to healthcare providers and insurance companies.
- Crisis intervention: Providing immediate support to patients experiencing emotional distress or challenging situations.
- Education and counseling: Offering individual or family counseling to address social and emotional issues impacting health.
- Collaboration: Working closely with the healthcare team, including doctors, nurses, and therapists, to ensure integrated care.
Experience
- Medical Case Management Experience
- Experience working with underserved populations, particularly Native American/Alaska Native communities, is highly preferred.
- Excellent interpersonal and communication skills to work effectively with diverse patient populations, families, and community organizations.
- Ability to work independently and collaboratively within a multidisciplinary team.
