```Job Summary```
We are seeking a skilled and compassionate Nurse Practitioner to join our healthcare team. As a Nurse Practitioner, you will provide high-quality patient care and support to our patients. Your primary responsibilities will include conducting diagnostic evaluations, performing laboratory tests, providing patient education, providing chronic care management, and collaborating with Hospice Medical Director on recertification process of Hospice patients. The ideal candidate will have experience in a variety of medical settings, possess excellent clinical skills, and be able to work effectively in a fast-paced environment.
```Duties```
- Perform comprehensive physical assessments on patients
- Order and interpret diagnostic tests
- Order medications and treatments as needed
- Provide patient education on disease prevention, treatment options, and medication management
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Monitor vital signs and assess changes in patient conditions
- Document all patient interactions accurately and thoroughly in the electronic medical record system
```Experience```
- Minimum of 1-2 years of experience as a Nurse Practitioner in an acute care or primary care setting
- Proficiency in diagnostic evaluation and interpretation of test results
- Experience with laboratory procedures and specimen collection processing
- Strong clinical skills and knowledge of evidence-based practice guidelines
- Familiarity with electronic medical record systems
