Responsibilities:
- As the Nurse Practitioner or Physician Assistant, you’ll be responsible for 6-7 home visits daily with our homebound patients
- Identify and address gaps in care and reconcile medications
- You’ll collaborate with other team members for care coordination, home health, nursing facilities, specialists and hospitals
- Find support through a scribe/medical assistant, nurse, social workers and more
Schedule: Monday – Friday, 8:00a – 5:00p
Salary: $120,000 to $140,000

Qualifications:
- Current PA NP or PA licensure
- 1+ year of experience required
- Home-based care experience highly preferred
- Bilingual preferred
