Asian Community Care Management (ACCM) is seeking a compassionate and experienced Registered nurse/ Care Manager to join our team. As an approved Utilization Review Agent under the New York State Department of Health, ACCM is dedicated to providing excellent care management services to our members. The position is fully remoted.
Essential Duties and Responsibilities
1. Assumes responsibility for reviewing and auditing admission documentation including clinical assessments/re-assessments to ensure that the Plan of Care adequately reflect and is appropriate to the assessment
2. Arrange and coordinate delivery of needed services.
3. Conducts telephone assessments, collecting sufficient data to make appropriate referral and certification decisions for emergency/urgent referrals that require alternate levels of care
4. Utilize strong clinical judgment, critical thinking and decision-making skills to gather and analyze medical, behavioral, social data from assessments, including medication and disease management.
5. Create, Monitor & ensure the delivery of a client centered plan of care identifying needed services, supports, and interventions.
6. Provide patient and family clinical education
7. Work with client and family members to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes.
8. Facilitate care planning as part of the interdisciplinary team
9. Evaluate and modify plans of care as required.
10. Establish and maintain liaison with nursing home, hospital discharge planners, human services providers, home care agency staff, physician offices, clinics and other community resources.
11. Assist other care managers (team SWCM) with questions resulting from reassessments and other situations in medically complex cases.
12. Monitor clinical outcomes – difficult to apply in remote care management. Foster high levels of client satisfaction.
13. Demonstrate effective skills in defining, verifying, and identifying ownership of issues and collaborate on resolving problems.
14. Participate in regular staff meeting and training events.
15. Perform other duties as assigned.
Job Type: Full-time
Pay: $90,000.00 - $95,000.00 per year
