Job Overview
This is a fully-remote role. We're seeking a dedicated Licensed Practical Nurse to join our distributed healthcare team. The ideal candidate will provide quality nursing care to patients in various healthcare settings. Our focus is providing care to patients on our care programs (such as; Remote Patient Monitoring and Chronic Care Management). You'll be working closely with a Physician implementing protocols for these care models. We're seeking folks who can build relationships with patients remotely and provide them with amazing care.
Responsibilities
- Perform patient assessments and provide direct patient care
- Administer medications and treatments as prescribed by the physician
- Assist in developing and implementing patient care plans
- Monitor and record patient vital signs
- Educate patients and their families on health conditions and treatment plans
- Assist with discharge planning and coordination of care
- Maintain accurate medical records
- Provide health coaching and support to patients
Qualifications
- Must have a current LPN license in good standing
- Experience in primary care, family medicine, and internal medicine.
- Proficiency in administering and managing medical records
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced and remote environment
- Knowledge of vital signs monitoring and patient assessment techniques
Job Types: Full-time, Part-time, Contract
Pay: $28.00 - $30.00 per hour
Expected hours: 40 per week
