Summary
RelateCare is a leading healthcare provider committed to delivering high-quality patient care through innovative telehealth solutions. We are seeking a dedicated and experienced Telehealth RN to join our team and support our Post Discharge and Pre-op Call Program.
As a Telehealth RN, you will play a crucial role in providing post-discharge and pre-operative care to patients through telehealth channels. This position involves assessing patient symptoms, providing education, triaging medical concerns, and ensuring a seamless transition from home to the hospital. Knowledge of pre-operative care and the ability to work with healthcare technology and informatics tools are essential for success in this role.
Shift details:
Lines open between 9am - 9pm PST, Monday to Saturday
Shifts looking to fill most urgently:
Part time: 2:00 - 6:00 /5:00 - 7:00 PM EST, with additional hours and rotating holidays
Full time: up to 40 hours per week, cover for the part time shift above
Essential Functions
· Post Discharge Follow-up
o Conduct post-discharge calls to patients to assess their health status.
o Provide education on post-discharge instructions and medication management.
o Triage patient symptoms and escalate urgent issues as necessary.
o Coordinate with healthcare providers and resources to address patient needs.
· Pre-op Support:
o Assist patients in preparing for surgery by providing pre-operative care instructions.
o Educate patients on the pre-operative process, including preparations and expectations.
o Address patient concerns and questions related to surgery and anesthesia.
o Ensure patients are well-informed and prepared for their upcoming procedures.
· Technical Support:
o Utilize telehealth technology platforms for patient interactions.
o Troubleshoot technical issues related to patient equipment such as Innovo.
· Documentation and Reporting:
o Maintain accurate and up-to-date electronic health records for all patient interactions.
o Generate reports on patient outcomes, triage activities, and telehealth usage.
· Collaboration and Communication:
o Collaborate with other healthcare professionals, including physicians, nurse practitioners, and social workers, to ensure comprehensive patient care.
o Communicate effectively with patients, caregivers, and family members.
o Participate in regular team meetings and training sessions.
Qualifications:
· Active Registered Nurse (RN) license in California.
· Bachelor of Science in Nursing (BSN) preferred.
· Minimum of 3 years of nursing experience, preferably in case management or pre-operative care settings.
· Proficiency in telehealth technology, Electronic Medical Records and Informatics tools.
· Strong communication and patient education skills.
· Ability to provide compassionate care and triage patient symptoms effectively.
· Knowledge of pre-operative care protocols and hospital admission procedures.
· Excellent problem-solving and technical troubleshooting abilities.
· Commitment to patient privacy and confidentiality.
Reporting structure
· Directly reporting to the Nurse Lead
