Job description
The licensed nurse is responsible for all phases of direct resident care for assigned residents on his or her shift. Certain responsibilities for the direct care of each resident may be delegated to appropriate nursing personnel within the guidelines of the Nurse Practice Act. The licensed nurse is responsible to supervise nursing personnel to whom care tasks are delegated.
Receive/provide report at each shift change with updated information on the present status of each resident for continuity of care. Shift change report shall also include a controlled substance count and reconciliation between the oncoming and off-going nurse.
Provide care to residents in a person-centered manner, taking into consideration each resident’s rights, goals and preferences for care and accommodating the resident’s needs to the maximum extent possible and reasonable. This includes becoming familiar with each resident’s care plan in his or her medical record.
Administer all medications and treatments in accordance with the physician order for each resident on the assigned shift. Accurately document the administration of medications and treatments in the resident’s medical record, including refusals of care.
Monitor and evaluate each assigned resident’s response to current medications and treatments. Notify the resident’s provider as indicated with any new or concerning observations/findings.
Enter new physician orders into the resident’s medical record as soon as practical.
Order or reorder medications and other supplies using the facility’s designated process.
Keep medication and treatment carts clean and properly stocked at all times, including labeling medications with open/use by dates when indicated.
Communicate changes in resident condition to the director of nursing, resident’s provider and resident’s representative as indicated. This includes but is not limited to medical, cognitive or physical condition from their baseline, new skin conditions, accidents/incidents, changes in behavior, etc. which may require a change in the current plan of care.
Initiate an incident report for any incidents that occur for assigned residents during the shift, including all associated assessments and notifications.
Review and update the resident’s plan of care as indicated with changes in goals, problems, needs, approaches and interventions.
Maintain completeness of each assigned resident’s medical record by completing all assigned assessments and progress notes according to posted schedules for the shift, making sure entries are timely, informative and accurate.
Collect specimens for laboratory testing in accordance with physician orders, fill out necessary lab requisitions, report results to the provider and complete applicable documentation in the medical record.
Assist with the admission/readmission of residents to the facility, including obtaining a report from the previous care setting, reviewing admission orders with the resident’s provider and then entering admission orders as indicated, welcoming the resident, orienting the resident to the facility, completing initial admission assessments, reviewing applicable consent forms, completing an admission progress note, filling out initial laboratory requisition form, etc.
Assist with the discharge of residents from the facility, including preparing discharge paperwork and completing discharge education with the resident as indicated, preparing medications for discharge, etc.
Collaborate with other facility departments and ancillary service providers (pharmacy, lab, etc.) to coordinate provision of care.
Help maintain cleanliness of work areas, break areas, linen closets, supply closets, etc.
Perform other related duties as may become necessary or as directed by supervisor(s).

Pay: $43.00 - $47.00 per hour
Expected hours: 20 – 40 per week
