We specialize in the care of complex patients and strive to deliver expert, personalized care to each individual. Out in the field, you will work closely with Skilled Nursing Facilities, Assisted Living, Home Health, and Hospice staff in ensuring the health and safety of our patients.

Responsibilities Include:
Performing face-to-face encounters with the patients.
Collecting and assessing patient care data, developing and implementing a plan of care, managing and evaluating the outcome of care.
Providing patients and/or families with health education, counseling, and referrals to other healthcare professionals and community resources when appropriate.
Participating in case management.
Ordering treatments and durable medical equipment as indicated.
Administrative work is completed onsite or at your home office.

· Works well independently
· Timely with documentation
Qualifications
Fully licensed in the state of Idaho

Pay: $75,000.00 - $120,000.00 per year
