Job Summary:
We are seeking skilled and compassionate RNs, LPNs, NPs, and DOs to deliver high-quality, patient-centered care in a fully virtual environment. The ideal candidates will be self-motivated, tech-savvy, and dedicated to excellence in healthcare delivery.
Key Responsibilities:
- Conduct virtual consultations, assessments, and follow-ups using telemedicine platforms.
- Provide patient care for a range of services, including urgent care, chronic condition management, behavioral health counseling, and wellness programs.
- Develop and implement personalized care plans in collaboration with patients and other healthcare providers.
- Utilize electronic health record (EHR) systems to document patient interactions and maintain compliance with all regulations.
- Monitor patients remotely using virtual facility monitoring tools and respond to alerts or concerns as needed.
- Educate patients and families about health conditions, medications, and preventative care strategies.
- Participate in team meetings, training sessions, and quality improvement initiatives to enhance service delivery.
Qualifications:
- Active licensure as an RN, LPN, NP, or DO in the state of operation.
- Previous experience in urgent care, primary care, hospice, behavioral health, or similar settings is preferred.
- Familiarity with telemedicine platforms and EHR systems, such as RXNT and Doxy.me, is a plus.
- Strong communication and interpersonal skills to effectively engage with patients and team members in a virtual setting.
- Ability to work independently, adapt to new technologies, and maintain patient confidentiality.
