Job Title: Remote Care Manager (LPN )
Location: Remote
Hours: Monday - Friday, 9:00 AM - 5:00 PM
Position Overview:
We are seeking a dedicated and experienced Care Manager (LPN) to join our team. This remote role involves creating comprehensive care plans for patients in long-term care settings, conducting monthly chart reviews, and addressing care gaps.
Key Responsibilities:
- Care Plan Development: Design and implement individualized care plans for patients in long-term care, ensuring that all aspects of patient care are addressed and documented.
- Monthly Chart Reviews: Review and update patient charts on a monthly basis to monitor progress, address care gaps, and adjust care plans as necessary.
- Vacation Planning: Coordinate and manage vacation plans for patients, ensuring continuity of care during these periods.
- Telemedicine Visits: Manage and coordinate telemedicine visits, ensuring that all technological and clinical aspects are handled efficiently.
- Progress Monitoring: Track and evaluate patient progress on care plans, making adjustments as needed to meet patient needs and improve outcomes.
- Care Gap Analysis: Identify and address any care gaps, working collaboratively with other healthcare professionals to ensure comprehensive patient care.
Qualifications:
- Licensure: Licensed Practical Nurse (LPN) with current and valid certification.
- Experience: Minimum of one year of experience in long-term care settings.
- Skills: Strong organizational and communication skills, proficiency in remote care management tools, and the ability to work independently.
Preferred Qualifications:
- Experience with telemedicine platforms and remote care coordination.
- Familiarity with electronic health record (EHR) systems.
- Strong problem-solving skills and the ability to handle complex care scenarios.
Job Type: Full-time
Pay: $25.00 - $30.00 per hour
