Job description
Skilled clinicians play a crucial role in home healthcare, providing essential services to patients who are unable to leave their homes. These professionals adeptly blend clinical excellence with an understanding of community health needs, ensuring that each patient receives personalized care. By traveling to patients' homes, following the care plan put in place, and working closely with the patient’s physician and Care With Love’s clinical team, the skilled care clinician becomes a bridge to the broader healthcare community, enhancing the quality of life for individuals and families alike.
The Skilled LPN:
- Provides direct patient care as defined in State's Name Nurse Practice Act.
- Implements a plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to
- diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified
- needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or
- Clinical Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
Communication
- Prepare clinical notes and update the primary physician when necessary and at least every sixty days.
- Communicate with the physician regarding the patient's needs and report any changes in the patient's condition; obtain/receive physician's orders as required.
- Participates in the peer consultation process. Communicate with the Clinical Manager and other clinicians on the team to provide updates, insight, and recommendations.
- Communicate with community health related persons to coordinate the care plan
Qualifications
- Graduate of an accredited practical nurse or vocational nursing program and is currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Has three years nursing experience. Community/home health or medical/surgical experience is preferred.
- Demonstrates excellent observation, problem solving, verbal and written communication.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possesses and maintains current CPR certification.
- Can provide a copy of a negative TB test.
- Self-directed and able to work with minimal supervision. Shows ability to organize and prioritize workload independently.
